Energy Assistance Program

The Wisconsin Home Energy Assistance Program provides assistance for heating costs, electric costs and energy crisis situations. The program begins 10/1/18 and ends 5/15/19. To apply, call the Energy Assistance Hotline at 715-485-8480. For more information regarding the program, click on the link below.



The Polk County Health Department, in collaboration with Polk's three medical centers, the United Way St. Croix Valley, and UW-Extension is conducting a community survey to determine the most important health issues in Polk County. Some of you may have taken this survey in 2015. The information provided will be used to assess if our health is improving or if new health challenges are occurring. Your participation is voluntary and will not affect your health benefits or healthcare in any way. All responses are completely confidential.


At the end of this survey, you have the option of entering your name for a drawing for a $100 fitness tracker, a $25 gas card or a $10 Subway card. Your name will not be linked to your survey.
